What are the consequences of an incorrect listing?
Case in point: Britain's Piper Alpha is one of the north sea's most productive oil RIGS. On July 6, 1988, an explosion occurred suddenly, killing 167 people within 45 minutes. The fire lasted 36 days.
Cause of accident: a pump which is not isolated by the energy, which has been lockout tagout and removed from the safety valve is started as a backup pump, and the liquefied petroleum gas leaked from the blockboard, causing an explosion.
Improved: in the third year after the disaster, a new rig, the braver, was built near the original alpha platform. The platform was installed with a more complete energy isolation system, strictly implemented the lockout tagout procedure and license system, and became the safest drilling platform in the north sea at that time.
