Ten percent of safety incidents are due to incorrect use of LOTO;
More than a quarter of a million workplace accidents are associated with LOTO each year.
OSHA standard 29CFR1910,147 requires that locker-tag control be performed on machinery and equipment when an employee must remove or bypass protective devices or place any part of his/her body in the equipment area during installation, maintenance, repair, clearing, adjustment, inspection, operation, processing, or construction.
